Fantastic web site of the Disabled Musicians' Coalition that is practical and easy to use. Full of ideas on adapting instruments, stands, chairs etc. A firmly musician centred approach.
Accessible Music
Dutch web site that has been established as a comprehensive resource for all those interested in the future of accessible music and emerging multimedia technologies. They aim to cover some of the more technical aspects of music notation and music coding but also want to provide practical information on making music accessible for everyone.

Yorkshire Association for Music and Special Educational Needs, based in Leeds, explores the role of music for communication, education and pleasure. Runs workshops and courses and has an instrument library.
National Disability Arts Forum
National organisation for disability and arts, they will send you a weekly bulletin EtCetera for free which has loads of interesting stuff, including a what's on TV and radio for the coming week which has relevance.
The Swarthmore Centre
and
Leeds College of Music
both have music courses for people wth learning disabilities.
